The general information here will help us begin to form a worship team database, but the more personal information will be kept between you and me for my prayer list.  Thanks in advance for your transparency.

  1. What's your best range to sing?  (Soprano, etc.)

 

  1. Rate your sight reading on a scale of 1-10?

 

  1. Rate your "ear" (picking up melodies, harmonies) on a scale of 1-10?

 

 

For Musicians:

  1. What instrument(s) have you played in praise and worship? 

 

  1. What instrument do you most enjoy playing?

 

  1. Rate your sight reading on a scale of 1-10?

 

    7.      Rate your "ear" (picking up chords, rhythms, breaks, etc.) on a scale of 1-10?

 

 

For Both:

 

  1. Tell me how long you’ve attended CHEF and what part you’ve played in praise and worship here.

 

 

  1. Are there other areas in which you serve at CHEF?   What are they?

 

 

 

  1. Musically (not necessarily spiritually) what artists or styles have most influenced your ideas about music?  (No one will get in trouble for how they answer this, unless your answer is "Weird" Al Yankovic.)

 

 

 

  1. Do you listen to contemporary praise and worship? (I don't mean contemporary Christian music radio music, but actual live worship recordings like Vineyard, Worship Together, Passion, Maranatha, Hosanna Integrity, etc.)

 

 

 

  1. How many weeks per month (and which weeks) are you willing/able to devote to helping lead praise and worship?  Will your family support your time away from them? 

 

 

 

  1. Do you have a sense of "calling" regarding praise and worship?  If so, have you limited other activities in your life to allow you to take a more regular role on the worship team? 

 

 

 

  1. Are you currently a CHEF Member or somewhere in the process of membership?

 

 

  1. Do you currently have a regular prayer partner?

 

 

  1. In what area(s), either spiritually or musically, would you most like to grow?

 

 

  1. Do you know your spiritual gift(s)?  Your temperament type?

 

                       

  1. Which Scripture passages have spoken most clearly to you about praise and worship? 

 

 

  1. The primary goal of this worship ministry is to foster an atmosphere where God and His people can touch each other.  Will you commit to pray daily and ask God to accomplish this?
